Preparing for Admission
Thank you for considering Cedar Mountain Center at Cody Regional Health. Our goal is to help make the admission process as smooth as possible. Below you’ll find important details about the application process, required documentation, and what to bring when you arrive for treatment.
Admission Requirements
Cedar Mountain Center requires the following items to begin the application process:
- Application for Admission – A completed application form must include a Letter of Intent describing your need for treatment.
- Addiction Severity Index (ASI) – This assessment must indicate a need for treatment at an ASAM Recommended Level of Care 3.5.
- Financial Documentation for State-Funded Beds – Applicants seeking a state-funded bed must submit bank account information with their application to verify eligibility.
Please note:
All applications are reviewed by our clinical and medical treatment teams
before any offer of admission is made. Completing an application does
not guarantee a bed date.

What to Bring

What to Bring & What Not to Bring (PDF)
Highlights include:
- Personal hygiene items (non-aerosol only)
- A week’s worth of modest, comfortable clothing
- Tennis shoes and appropriate workout attire
- A small amount of spending money for laundry and personal needs
Please do not bring:
- Electronics (phones, smart watches, tablets, etc.)
- Perfumes, aerosols, or scented lotions
- Books or magazines other than a Bible or journal
- Tobacco products

Medication & Health Information
Please bring:
- A 60-day supply of all prescribed medications
- A list of all medications from your pharmacy
- Any necessary medical equipment (e.g., oxygen, CPAP)
- Nicotine replacement products (patches or gum), if applicable
Communication & Visitation
- Phone Calls: Allowed after 14 days in treatment (Tues., Thurs., Sat. – 10 minutes per call)
- Mail: May be sent or received after 14 days and will be opened in front of staff
- Visitation: Allowed on Sundays, 1:00 PM – 3:55 PM, after 14 days
